Featherdown's Quick Look extension registers the first time you launch the app — so open Featherdown once after installing. If a preview still looks wrong, log out and back in, or run qlmanage -r && qlmanage -r cache in Terminal to reset Quick Look.
Open Featherdown's Settings (⌘,) and click Make Featherdown the Default. Every .md file will then open in Featherdown on double-click.
Featherdown looks for images next to your note and then walks up parent folders to find an images/ folder — the common Obsidian layout. If an image lives entirely outside your home folder, the Finder preview shows a placeholder chip; open the file in the app to view it.
